Teo Korean Medicine Clinic Bundang Forward head posture / Straight neck

30-Second Answer

A person sitting in front of a laptop with the head jutting forward

When you receive care for straight neck or forward head posture, we first check the chief complaint, when and why it began, and what makes it worse.
We check tenderness and range of motion, cervical and thoracic displacement and asymmetry, and tenderness at each spinal segment.
We look at the impact of the load on the neck together with pelvic height, left-right spinal misalignment, shoulder height, and shoulder girdle alignment.
We also check the distribution of nerve symptoms, such as numbness in the arms or legs.

Please check first

If neck pain begins after an injury such as a fall or a traffic accident, imaging may be needed first to check for cervical damage such as a fracture. If numbness in the hands, weakness, or unsteady walking are present as well, other causes such as myelopathy must be checked first. If a younger person has continuing neck or back stiffness that is worse at night and eases with movement, an examination is needed to check for an inflammatory spinal condition.

Common questions

We look separately at posture problems, nerve symptoms, and the order of tests.

Are forward head posture and a cervical disc the same?

Forward head posture and straight neck are assessed mainly around the forward head position and neck alignment. For cervical disc herniation, how far neurological symptoms spread, such as numbness or pulling running from the neck into the arm and hand, is an important item to check. Not everyone with forward head posture has a cervical disc herniation, and the two can appear together, so we do not name a condition from posture alone.

What else do we look at if there is arm numbness?

Please tell us whether there is only stiffness in the neck and shoulders, or whether numbness or weakness reaches the arm and hand. We also check which arm and which fingers are uncomfortable, whether it changes with neck movement, and whether it appears in both hands. If there is numbness in the arm, we do not pass it off as a simple posture problem but look at the distribution of nerve symptoms as well.

Do I need an MRI or X-ray first?

Not every case of forward head or straight neck needs imaging first. If neck pain began after a fall or a collision, imaging may come first to check for cervical injury. If hand numbness, weakness, or unsteady walking are present together, or severe stiffness continues at night, myelopathy or another cause should also be checked first.

Is correcting the neck alone enough?

In examination we check not only cervical and thoracic displacement but also pelvic height, left-right rotation of the spine, shoulder height, and shoulder-girdle alignment. Repeated habits such as sitting for long periods, looking down, and resting the chin on the hand can also be related to the load on the neck. After treatment we check again how the discomfort and the symptoms have changed and adjust the plan.

FAQ

What is checked in care for forward head and straight neck posture?

We check the chief complaint, when and why it began, what makes it worse, past medical history, history of operations, current medications, tenderness, range of motion, pelvic tilt and leg length difference, cervical and thoracic displacement and asymmetry, tenderness at each spinal segment, and the distribution of nerve symptoms.

What is checked in body alignment correction?

We check pelvic height, left-right spinal twist, shoulder height, and shoulder girdle alignment.

In what order is Chuna manual therapy performed?

We proceed in the order of pelvis and legs, lumbar spine, thoracic spine, and cervical spine, adding or skipping parts as needed.

What is the treatment schedule?

For about the first 1 week, sessions are 3 to 5 times a week; in weeks 2 to 3, every other day for 3 times a week; and after that, 2 times a week. If correction is the goal, it is 3 times a week at first and 2 times a week after that.

What is checked on a return visit?

We review discomfort and symptom changes after the previous treatment and plan the next treatment.

What is the difference between forward head posture and a cervical disc?

For forward head and straight neck posture we focus on neck alignment and posture, while for a cervical disc the distribution of nerve symptoms, such as numbness or pulling spreading to the arm, is a key item to check. The two can appear together, so we do not tell them apart by posture alone.
